Opportunity Healthcare, a leading locums and placement firm, is at the forefront of “Connecting the Best in Healthcare.” We are currently facilitating the search for a dedicated Advanced Practitioner to help a compassionate healthcare facility South of Toledo. 

  
Your New Role: 

Position: Palliative Care and Hospice Advanced Provider 

Job Duration: Full Time/Permanent 

Setting: 100% Inpatient 

Schedule: 

5-day work week 

8:00 AM – 5:00 PM 

Participate in call rotation 

Experience: 

Required: 3+ years experience as an advanced provider in an acute care setting or with palliative care and hospice 

Must be Geriatric certified or obtain certification within 1 year 

Patient Population: 

Inpatient hospice patients 

Patients requiring palliative care and chronic disease management

In this Role: 

The Palliative Care and Hospice Advanced Provider will deliver comprehensive care to inpatient hospice patients, focusing on symptom management, chronic disease management, and advanced care planning. Responsibilities include assessing, planning, implementing, and evaluating patient care needs in line with the mission of palliative care. The provider will round on inpatient hospice, participate in call rotation, and work collaboratively with the healthcare team to develop and implement care plans. This role requires a commitment to patient-centered care and excellent communication skills to support patients and their families through advanced care planning and end-of-life care decisions. The provider must be geriatric certified or willing to obtain certification within a year.
