Oklahoma RN License by Endorsement Process – Step by Step

1.) Verification of RN Licensure from the Original State to Oklahoma for Endorsement Proof

1. If your license state is a participant of Nursys (Michigan/Pennsylvania are not), use the following link:
   
   1. https://www.nursys.com/NLV/NLVTerms.aspx
   2. Pay $30 to send the verification to Oklahoma.
2. If the state does not participate in Nursys verifications, you must request a verification directly from the state of licensure to the Oklahoma BON.

2.) Submit Official Transcripts

1. Send official transcripts from your nursing school to the Oklahoma Board of Nursing.
   
   1. You can send transcripts to the following address electronically by using the sites below:
      
      1. www.parchment.com
      2. https://www.studentclearinghouse.org
2. Have transcripts mailed to the OK Board of Nursing.

3.) Complete the Online Oklahoma RN License by Endorsement Application

1. https://okbn.boardsofnursing.org/okbn/
   
   1. Create Free Account.
   2. Click on Apply for License at the upper right area.
   3. Select the License Application Option that is appropriate to you.
   4. Request a Temporary License to obtain a license faster – $10.
      
      1. Temp Licenses are valid for 90 days.
      2. Not issued for those with arrest records.
   5. Complete all Sections.
   6. Pay $85 and submit application.

4.) Background Check – Fingerprints

1. IN STATE (OK mailing address):
   
   1. Register to obtain fingerprints via an IdentoGo center within Oklahoma: https://ok.ibtfingerprint.com/ – register under the OK BON.
      
      1. ENTER THE Service Code – 287N8H.
      2. Make an appointment.
      3. Complete your fingerprints.
2. OUT OF STATE (other state mailing address):
   
   1. Visit the site below to find a digital fingerprint location near you: https://ok.ibtfingerprint.com/
      
      1. Click on the green Box Towards the bottom of the screen and click “Register for Out of State Digital Fingerprint Service”.
      2. ENTER THE Service Code – 2B7N8H.
      3. Follow the instructions to make an appointment.
         
         1. If no locations are found, you will have to request a packet with fingerprint cards be mailed to you from the Oklahoma Board of Nursing.
         2. Once received, visit the aforementioned site and select “register for fingerprint card process service”.

5.) Verification of Continuing Qualification for Practice

1. Employer Signed document confirming at minimum 520 hours of employment in the last 5 years prior to applying for OK Licensure by Endorsement.
   
   1. Download from the following site(s):
      
      1. https://nursing.ok.gov/empveri.pdf
      2. OR
      3. Employment Verification Form.pdf
   2. If you have multiple employers needing to complete the form to make up your 520 hours, please send each employer 1 form.
2. Upload the Form to your Oklahoma License Account.

6.) Evidence of Status Form

1. Fill out the Form.
   
   1. Download from the following site(s):
      
      1. https://nursing.ok.gov/evidstatus.pdf
      2. OR
      3. Oklahoma – Evidence of Status Form.pdf
2. Have the form Notarized.
3. Upload the form to your Oklahoma License Portal ALONG with a photocopy of your citizenship document.
